In 2003, Dr. Héctor David Acosta Hernández founded Hospicio Las Brisas to provide palliative care services to patients at their homes. He provided those services to patients in town of Vega Baja and its vicinity. The company started to stand apart from the rest because of its high-quality services and the dignified care provided to patients and their families. The company served the community of older adults, without excluding any patient with a terminal illness.
As a member of the American Academy of Hospice and Palliative Medicine, Dr. Acosta made sure that Brisas was always updating its protocols and service offerings to keep up with the latest advancements in palliative care. He also established rigorous hiring and training protocols for his employees, thus guaranteeing the consistency and quality of the services.
Today, Hospicio Las Brisas has changed its name to Brisas Health to embody the new range of services it offers. In the hands of David Acosta, who grew up immersed in the industry and shares his father's vision of service, the company continues to operate with the same professionalism, humane values and commitment that have distinguished it for the past 25 years.
Who is eligible for these services?
Patients are eligible for hospice care when:
– A doctor (if they have one) and the medical director of hospice care certify that the patient is terminally ill and that he/she probably has less than six months to live.
– The patient signs an agreement choosing hospice care instead of the ordinary benefits for his terminal illness.
Who is eligible for these benefits?
The patient is elible for palliative care when:
-They have an advanced disease or a diagnosis with a high risk of life-limiting morbidity and mortality.
-They do not have a predictable forecast.
-Are not eligible nor interested in choosing formal hospice services.
-Is confined to the home, commonly due to multiple chronic diseases, including cognitive impairment with functional limitations and decrease.
-They experience significant symptoms associated with advanced illnesses.

Services that are covered
• Medical Services – are offered with a minimum frequency of 1 visit per month and whenever necessary, according to how critical the patient’s condition is. Services include management of vitals, symptoms and pain relief so that the patient can experience a better quality of life.
• Nurse Services – to evaluate and asses current vitals and symptoms, and monitors them with a minimum frequency of 1 visit per week, up to 2 to 3 weekly visits, according to the patient’s condition and needs.
• Social Services – assesses psychosocial needs that affect or prevent establishing the patient care protocol, and then offers the necessary support and resources. Minimum visits – 1 per month or according to the needs of the patient/caregiver.
• Chaplain Services – evaluates the spiritual needs of the patient and/or caregiver that affect the acceptance process of the patient’s health condition, in order to offer the necessary support. Minimum visits – 1 per month or according to the needs of the patient/caregiver.
Support Services, According to Need:
• Hospice Assistant – offered as a support resource to the caregiver for help with patient’s hygiene/bath. It is offered from 2 to 5 visits per week or as needed.
• Housekeeper – available as a support resource to the caregiver to help with chores and patient care such as, bathroom assistance, cleaning the patient’s room, washing clothes, preparing meals for the patient, amongst others, according to need. This service is evaluated by social worker to determine the frequency that it will be assigned according to the needs of the patient and their caregiver.
• Speech Therapy and Physical Therapy – are offered when ordered by the doctor, for dealing with pain relief or other symptoms related to the terminal illness.
• Nutritionist – by medical order according to the patient’s need.
EQUIPMENT, MEDICAL SUPPLIES AND COVERED MEDICATIONS
Durable Medical Equipment:
The durable medical equipment within the scope of coverage must be ordered by the physician and in accordance to the patient’s needs to relieve and manage his condition. Delivery will be prioritized according to the date it was ordered.
